Don't use the template/sample. Trust me on this one, it's index is not correct.

Start a new image, 6144x4096 in Gimp. Then change the mode from RGB to Indexed (under the Image menu). Select the dragon template and uncheck the "Remove unused colors from final palette" option.

Step-by-step:
1) Create a new image with size 6144x4096
2) In the image window menubar (not the tools window), select Image->Mode->Indexed
3) Select "Use custom palette"
4) Select the dragonmod11-gimp palette
5) Uncheck the "Remove unused colors from final palette"
6) Click OK

It should work correctly after this.

That image is not correct because it was made with a different index.
You have to use a template that was made with mod 11 or make one from scratch with the correct index.
